Canker Sore vs Cold Sore: How to Tell Them Apart
They look similar enough to be confused constantly, and they are not the same disease. A cold sore is a herpes simplex virus infection that erupts as a cluster of fluid-filled blisters, usually on the lip border. A canker sore is a non-infectious ulcer of the moist lining inside the mouth, with no virus behind it and nobody to catch it from. Everything else follows from that one difference.
Side by side
| Canker sore | Cold sore | |
|---|---|---|
| Cause | Non-infectious; multiple predisposing factors | Herpes simplex virus type 1 |
| Where it sits | Inside the mouth: cheek lining, inner lip, tongue, base of the gums | On the lip border, under the nose, around the chin |
| First appearance | An open ulcer, white or yellow center, red halo | A group of fluid-filled blisters that later crust |
| Warning before it shows | Usually none | Burning, itching, tingling or throbbing a day or two ahead |
| Contagious | No | Yes, until every sore has scabbed |
| Typical course | Pain eases in 7 to 10 days, healing in 1 to 3 weeks | Blisters clear in about 5 to 15 days |
| What treatment targets | Pain and inflammation | The virus, plus the pain |
Location is the fastest test
Canker sores develop on non-keratinized tissue — the soft, movable, permanently wet surfaces: inner cheek, inner lip, floor of the mouth, sides of the tongue. Cold sores prefer the opposite terrain, the keratinized skin at the lip margin and the face around it. If you can see the sore in the mirror without pulling your lip away from your teeth, it is far more likely to be a cold sore.
The tingle belongs to only one of them
Most people who get cold sores recognize the prodrome: a day or two of burning, itching, stinging or numbness in the exact spot before anything is visible. That window is when over-the-counter antiviral medicine works best. Canker sores rarely announce themselves; some people report vague oral discomfort first, but there is no reliable warning phase and no antiviral to start.
Why the mix-up has a cost
Two costs, in fact. Treating a canker sore as herpes means buying an antiviral that cannot work and skipping the topical anesthetics and antimicrobial rinses the American Dental Association recommends. Treating a cold sore as a canker sore means skipping the precautions: the virus spreads by kissing, shared cups, shared towels and shared lip balm, and it is genuinely dangerous around newborns and anyone immunocompromised. More than half of Americans aged 14 to 49 already carry the virus, which is why it circulates so quietly.
When to have either one looked at
Any mouth sore still present after two weeks deserves an exam. So does a cold sore near the eye, which the AAD treats as an urgent problem because the virus can reach the eye itself. Repeat canker sores are worked up differently — the pattern that matters there is covered in recurring canker sores causes.

Frequently asked questions
Is one of them contagious?
Only the cold sore. It is caused by herpes simplex virus and the American Academy of Dermatology considers a person contagious until every sore has scabbed over — no kissing, no shared lip balm, towels, razors, drinks or utensils in the meantime. Canker sores carry no such precautions.
Can a canker sore appear on the lip?
On the inner lip, yes — the moist lining is a classic site. On the outer lip, the dry vermilion you can see when your mouth is closed, essentially no. A blister on that border is far more likely to be a cold sore.
Will antiviral cream do anything for a canker sore?
Nothing. Antivirals act on herpes simplex, and a canker sore has no virus for them to act on. It is the most common wasted purchase in this confusion, and it delays the measures that actually reduce the pain.
Can I have both at the same time?
Yes, since they are unrelated conditions with unrelated triggers, although the combination is uncommon. Ironically, both can follow the same stressful week, which is one reason people assume they are the same thing.
Why is one called a herpetiform ulcer if it is not herpes?
Because of appearance only. Herpetiform aphthae are a canker sore subtype that arrives as dozens of tiny 2 to 3 mm ulcers, and StatPearls is explicit that the name comes from the resemblance to herpetic stomatitis and that there is no connection to herpes viruses.
