Exercise-Related Muscle Cramp Causes

Cramps during exercise — exercise-associated muscle cramps, or EAMC — have two competing explanations, and they are not equally supported. The dehydration and electrolyte-depletion theory is the familiar one. The altered neuromuscular control theory is the one the published evidence backs.

The two theories, side by side

Dehydration and electrolyte depletion. The intuitive story: sweat drains fluid and sodium, the muscle misbehaves. Schwellnus, reviewing the evidence in the British Journal of Sports Medicine in 2009, found that support for it came mainly from anecdotal clinical observations, case series totalling 18 cases, and one small case-control study of 10 people — while prospective cohort studies contradicted it.

Altered neuromuscular control. Fatigue disturbs the balance in the spinal reflex arc: input from the muscle spindles stays excitatory while inhibitory input from the Golgi tendon organs drops, and motor neurons keep firing. Support here comes from human cramping models, epidemiological studies in cramping athletes, and animal experiments.

The 2022 evidence-based review in the Journal of Athletic Training, co-authored by Schwellnus, concluded that alterations in neuromuscular excitability and, to a much lesser extent, dehydration and electrolyte losses are the predominant factors. Two findings drove that: plasma characteristics were often within normal limits and comparable between athletes who cramped and those who did not, and stretching relieved the cramping without altering fluid or electrolyte levels.

How often athletes cramp

StatPearls reports prevalence of 67% in triathletes, 30% to 50% in marathon runners, 52% in rugby players and 60% in cyclists. Risk rises with prolonged high-intensity exercise in hot environments — and the strongest single predictor is having cramped before. The general mechanism behind all cramping is covered in muscle cramp causes.

What to do when one hits

Apply gentle static stretching to the cramping muscle until it abates. That is the acute treatment carried at the strongest evidence grade in the 2022 review. Quinine is explicitly recommended against. StatPearls adds massaging the muscle and, for exercise-induced cramps, rehydrating and replacing sodium losses.

Prevention

The 2022 review does not offer one universal rule — it recommends a targeted approach built on the individual athlete’s own risk factors. Strength training and training under competition-like conditions both carry moderate-strength support. StatPearls frames prevention as reducing premature muscle fatigue: adequate hydration, conditioning, heat acclimatization, a diet that matches the training load, and electrolyte replacement in high-sweat situations. Whether mineral supplements help at all is a separate question — see muscle cramps and magnesium or potassium deficiency.

When to see a doctor

Get evaluated if exercise cramps are frequent despite good conditioning and hydration, or if they come with persistent muscle pain, weakness, swelling or dark-colored urine after workouts — that combination can signal rhabdomyolysis and needs prompt attention.

Frequently asked questions

Why do cramps happen during exercise?

Two theories compete. The dehydration and electrolyte-depletion theory is the popular one; the altered neuromuscular control theory is the one the evidence supports. The 2022 Journal of Athletic Training review concluded that changes in neuromuscular excitability and, to a much lesser extent, fluid and electrolyte losses are the predominant factors.

Does drinking sports drinks prevent cramping?

It is not the settled answer people expect. Studies found plasma electrolyte concentrations often within normal limits and comparable between athletes who cramped and those who did not, and sodium supplementation did not differ between ultramarathoners with and without cramps. StatPearls still advises replacing fluid and sodium losses, especially in the heat.

What should I do if a cramp happens mid-workout?

Stop and apply gentle static stretching to the cramping muscle until it settles — the only acute treatment the 2022 review graded with the strongest level of evidence. Notably, stretching relieves the cramp without changing fluid or electrolyte levels at all.

Who is most likely to cramp during exercise?

Endurance athletes, especially during prolonged high-intensity effort in the heat. Reported prevalence in StatPearls includes 67% in triathletes, 30% to 50% in marathon runners, 52% in rugby players and 60% in cyclists. A previous episode is the single strongest risk factor.
