Pain Between Shoulder Blades: What It Could Be
The strip between the shoulder blades is where posture accumulates. Most pain there is muscular: the mid-back holding a forward lean for hours, an overhead lift, a bag on one side. But this region is also where the heart and the aorta refer pain, which is why sudden and severe belongs in a different category from persistent and achy.
The common causes
- Sustained forward posture. A trunk leaning toward a screen, a head tipped down over a phone. The muscles between the shoulder blades work isometrically the whole time.
- Muscle strain. A single awkward reach, a heavy lift with the arms away from the body.
- Joint irritation in the thoracic spine and at the joints where the ribs attach.
- Uneven load. A shoulder bag or backpack carried on one side, day after day.
- A neck problem felt lower down. MedlinePlus describes cervical disc herniation as producing pain on moving the neck and deep pain near or over the shoulder blade — a genuine referral, not a coincidence.
Muscular pain here is tender to press, changes with position, eases with heat, and has a story you can trace to the week you had.
When it comes from the chest, not the back
Two cardiovascular emergencies present in this exact spot, and both have signatures.
Heart attack. The NHLBI lists chest pain, heaviness or discomfort along with pain or discomfort “in one or both arms, your back, shoulders, neck, jaw, or above your belly button,” plus shortness of breath, unexplained heavy sweating, nausea, light-headedness, and unusual tiredness for no reason — that last one more common in women. The NHLBI also warns that symptoms can be mild or absent, particularly in older adults and people with diabetes.
Aortic dissection. MedlinePlus describes pain that begins suddenly and severely, is sharp, stabbing, tearing, or ripping, and is “felt below the breast bone, and then moves under the shoulder blades or to the back.” It can travel on to the shoulder, neck, arm, jaw, abdomen, or hips, with fainting, heavy sweating, clammy pale skin, and a rapid weak pulse.
The distinguishing feature is not location. It is onset and company: instant, severe, and arriving with sweating, breathlessness, or faintness. That is a 911 call, not a heating pad.
Digestive causes worth knowing
Gallbladder pain refers classically to the right shoulder blade, and reflux is sometimes felt through to the mid-back. Both tend to follow meals rather than movements, which is the clue: if the pain has a relationship with dinner and none with your desk chair, the muscle is not the problem.
What helps the muscular kind
- Fix the setup. Screen at eye level, forearms supported, feet flat.
- Interrupt long sitting every 30 to 45 minutes.
- Superficial heat over the tight band.
- Strengthen the mid-back and shoulder blade muscles, so posture stops being held by endurance alone.
- Even out what you carry — two straps instead of one.

Go to the emergency room if
- The pain started suddenly and at full intensity, or feels tearing or ripping.
- There is chest pain, pressure, or tightness, even mild.
- There is shortness of breath, at rest or with almost nothing.
- There is cold sweat, nausea, or light-headedness.
- The pain radiates to an arm, the neck, or the jaw.
- You feel like you might pass out.
For persistent rather than sudden pain, NIAMS sets the bar at more than a few weeks, or any back pain arriving with numbness and tingling, weakness, fever, or unintended weight loss.

Frequently asked questions
Is pain between the shoulder blades usually serious?
Usually not. Most of it is muscular or postural and tracks with how you have been sitting, lifting, and sleeping. The exceptions are sudden, severe, or accompanied by chest symptoms, and those are urgent rather than merely worrying.
Can a neck problem cause pain between the shoulder blades?
Yes. MedlinePlus describes a cervical herniated disc as causing pain when moving the neck and deep pain near or over the shoulder blade, so the pain you feel in the mid-back can originate several levels higher.
Why does it hurt between my shoulder blades after eating?
Gallbladder pain classically refers to the right shoulder blade, and reflux can be felt through to the mid-back. If the pattern is reliably tied to meals rather than to posture, it is worth investigating the gut, not the muscle.
Is this pain different in women?
The muscular causes are the same. What differs is heart attack presentation: the NHLBI notes unusual tiredness lasting days is more common in women, and pain can show up in the back, jaw, or above the belly button rather than as classic chest pain.
How long should I wait before getting it checked?
NIAMS advises evaluation if back pain lasts more than a few weeks or arrives with numbness, weakness, fever, or unexplained weight loss. Anything sudden, severe, or breathless should not wait at all.
